I think you should add Kitty to the poll. After all, she went to the OP, too, and Bart already appeared in her form to annoy Nathaniel.
In the absence of Kitty, I agree with Ptolemy. I think Bart really loved him.
After a few houndred years he will choose Nat's form once or wice... But not as many times as Ptolemy's

Indeed. Nathaniel was noble towards the end, but Ptolemy was nice to Bart all the time they knew each other.

Ptolemy might have started out caring and kept on to the end caring, but that doesn't make him better than Nat. Nat has cared about Bart, even just a little, all along. Why else would he save weak frog Bart instead of extracting information from him at the very first second? And I think Nathaniel did a bit more than learning a lesson. He grew and realized what kind of a person he could and wanted to be. He sacerficed his lives for hundreds of people, possibly thousands of the demons would have gotten around the world. He didn't think of himself and his glory, but of defeating the demons and protecting others. And in the end, he demonstrated his love for Bart by letting him go.

Ptolemy easily. He liked Nat almost as much at Ptolemy at the very end, but he still didn't make the ultimate sacrifice. He would have had to do both to match Ptolemy. And have been nice to him erlier. He likes kitty loads too now because she made the sacrifice, but she wasn't his master, hadn't known him as long and didn't do the whole 'releasing just before death' thing. Didn't need to. But whatever, it will still be Ptolemy. Most of the time, anyway. Maybe to blend in sometimes, he will use Nat's younger form. But who knows?

But, between Nat or Ptolmey, Nat. I liked Nat better. Anyways, Bart never shared a body with Ptolemy. he never saw his thoughts. He never saw what a good, sweet person Ptolemy was...If he was a good, sweet person.

Ptolemy mainly, Kitty often, Nat somewhat.
Kitty and Nat together did the same as Ptolemy, but Ptolemy was just generally great to Bart, whereas the other two only "bonded" with bart when their whole world was disrupted anyway.

Faquarl (No, hear me out)
For all that Faquarl tried to kill Bart, in the last book Faquarl delayed killing him, allowing him to weaken in the fish soup toureen.
Thus, when he was summoned, he was weak enough to be ignored long enough to gather details on Nouda/Faquarls plan (With Faquarlie-warlie loudly proclaiming the details of his plan) to form a plan of his own.
IF Faquarl hadn't helped Barty like this, he would have been killed attacking Nouda under Natty boys charge: IF Bart hadn't saved Nat/Kitty, none of their repetition of Ptolemy's devoution would have occured!
NOUDA WOULD HAVE BEEN VICTORIOUS!
Thus: Faquarl was not just a rival but a KEY PLAYER in the eventual victory of Nat/Kitty over Nouda!
Barty, in gratitude to this carefully executed assitance by Faquarl, will now assume Faquarls form, and carry a Cleaver at all times as a mark of respect!

I loved Nathaniel and his death was so sad. I still cry about it sometimes.
So I think Bartimaeus will use Nat´s form. But of course he will never stop using Ptolemy´s form. He obviously loved him.
