North American news is frustrating. Who is actually in de facto control in Catalonia? Spanish authorities or Catalan?

That's a question we ask ourself daily...
Spanish authorities are nominally in control of catalonia, but catalan government does it best to obfuscate, misinterpret and ignore the higher ups. They take 'autonomous province' to the next level and distance theirselves from the rest of Spain. They still want their tourism and rent though.

The separatist movement seems to have spectacularly bungled things. Who declares independence without getting regional authorities to actually enforce it?

The catalonias since, like, always. They declared catalonian republic when the first spanish republic happened and they still do it now.
