In some cases the legitimate government is very hard to determine, exterminated, or directly at fault, so no, the UN doesn't require the legitimate government's permission/request.

The UN is kitted out for anti insurgency. It has no Airforce, no Armour, no Artillery. NATO is the de facto World Police Force.
